Botox vs Fillers: What’s the Difference?

“Botox” has become shorthand for almost any injectable treatment, but Botox and dermal fillers actually do very different jobs. Knowing the difference makes it much easier to tell your practitioner what you actually want, and to set realistic expectations for the result.

What Botox Does

Botox (and other botulinum toxin brands) works by gently relaxing the small muscles that cause expression lines, the ones that form when you frown, squint, or raise your eyebrows. Over time, repeated muscle movement is what etches lines like crow’s feet and forehead furrows into the skin. By softening that movement, Botox smooths existing lines and slows new ones from forming.

It’s best suited to the upper face: forehead lines, frown lines between the brows, and crow’s feet around the eyes. Results typically appear within 3–7 days and last around 3–4 months before the muscle activity gradually returns to normal.

What Fillers Do

Dermal fillers work differently, instead of relaxing muscles, they add volume. Most fillers use hyaluronic acid, a substance your skin already produces naturally, to fill in lines, restore lost volume, or reshape areas like the cheeks, lips, jawline, and under-eye hollows.

Where Botox addresses movement-related lines, fillers address static lines and volume loss, the kind of change that’s visible even when your face is at rest. Results are immediate and generally last anywhere from 6 to 18 months depending on the area treated and the product used.

Which One Is Right for You?

As a rough guide:

  • Lines that only show up when you make an expression (frowning, squinting) usually respond best to Botox.
  • Lines that are visible even with a relaxed face, or a loss of volume/definition, usually call for filler.
  • Concerns like thin lips, flat cheeks, or a soft jawline are filler territory, not Botox.

Can You Combine Them?

Yes, and it’s common. Many clients use Botox to soften expression lines in the upper face while using filler to restore volume elsewhere, sometimes in the same visit. A practitioner who assesses your face as a whole, rather than treating one line in isolation, will usually get you a more natural-looking result than either treatment alone.
